Dear Parents,
As students will be at school for their yearbook pictures, therapies, testing, etc. it is required for every student/visitor to undergo a symptom screening. Wentworth Jr. High is currently using this Covid-19 Self-Certification and Verification Form that is required to be used for students and visitors entering the school. To ensure compliance with IDPH Guidelines, sign and send with your student each day your student will be at school. Please call or reply with any questions or concerns.
